How long can Finasteride keep her: is it permanent?

Finasteride: The True Impact on Your Hair (And More)

Elke ochtend hetzelfde ritueel.

You're staring in the mirror. Counts the thin spots. Feel the panic coming on.

And you wonder, is finasteride the solution? Can I use it for years? What if I quit?

I get it. Hair loss eats away at your self-esteem.

Let's forget the marketing bullshit and look at the real facts. 👇

I've written several articles about Finasteride. Click on the link if you are interested or scroll to the bottom of the blog.

This video will explain everything you need to know about Finasteride.

What happens when you stop Finasteride?

Simply put: The protective shield disappears.

I saw it with my friend Thomas.

After three years of faithful intake, he thought, "Mission accomplished."

Big mistake.

Within 6 months of stopping:

  • His hairline crawled back
  • The crown became thinner
  • All the land won: lost

Finasteride is not a cure. It's maintenance.

Are you quitting treatment? Then the DHT returns. And your hair loss.

As predictable as the sunrise. ☀️

But how fast exactly?

Week 1-2: DHT levels are starting to rise Month 1: You may notice more hair in your shower Month 3: Visible change in hair thickness Month 6: Back to baseline (or worse)

Dr. Van Leeuwenhoek, a Dutch hair specialist, says: "Think of finasteride like a dam that stops DHT. Break the dam, and the river flows again."

Can Finasteride keep her forever?

Here comes the truth:

Nothing is permanent.

But... long-term use shows impressive results:

After 10 years of treatment:

  • 21% experience improved hair growth
  • 65% retains what they have
  • Only 14% see decline

That means 86% success after a decade of use.

Not bad for a little pill, right?

The secret? The first 12 months tell you everything.

See any improvement? Then you got golden long-term opportunities.

No improvement in year 1 = probably never result.

What makes those first 12 months so crucial?

Your hair growth runs in cycles. Finasteride should first lower the harmful DHT, then repair the follicles, and then start new growth.

This process takes time.

Let me take you into a typical timeline:

Month 1-3: Less hair loss (not always visible) Month 3-6: Stabilisation (you lose less) Month 6-9: First signs of regrowth Month 9-12: Visible results in responders

Martijn, 37, told me: "The first six months I saw nothing. I was about to give up. But in month 8 it all of a sudden started... as if someone had switched a switch."

Is Finasteride Permanent?

No. Absolutely not.

Finasteride works like a storm flood barrier. As long as it stands, your land stays dry.

But take him away...

DHT's coming back. And take your hair.

Here's how it really works:

  1. You take the pill
  2. It's blocking DHT (the "hair killer")
  3. Your hair gets breathing space to grow
  4. Are you stopping? The DHT flood returns

Don't expect miracles at week one. Or month one.

This is a marathon, not a sprint. 🏃‍♂️

A closer look under the bonnet:

Finasteride blocks the enzyme 5-alpha reductase. This enzyme converts your normal testosterone into DHT.

DHT binds to your hair follicles and causes "miniaturization" to be a gradual process in which strong, healthy hair transforms into thin, weak peach fuzz.

Finasteride reduces the DHT by about 70%.

This is enough to stop or even reverse the miniaturization process. But only as long as you use the medicine.

It's pure biochemistry. No magic.

Are There Permanent Side Effects?

Now it gets interesting. And personally.

The official figures: 2-3% experience sexual side effects.

For most: temporary.

For some: possibly permanent.

I know Pieter, 38 years old. Stopped after 8 months due to decreased libido.

"It took me four months to be myself again," he told me.

But then there's Mark, 42: "Two years stopped, still trouble."

Is Mark the exception? Probably.

But he exists.

And that makes the choice personal.

Let's be specific about possible side effects:

During use:

  • Libido decreased (1.8%)
  • Erectile dysfunction (1.3%)
  • Ejaculation problems (1.2%)
  • Depressive sensations (rare)
  • Pain in the testicles (rare)

After discontinuation (usually temporary):

  • Gradual recovery within weeks to months
  • 90-95% complete recovery within one year

After stopping (rarely):

  • The controversial Post-Finasteride Syndrome
  • Sustained sexual, cognitive and emotional problems
  • Unknown mechanism (hormone receptor damage? neurosteroid disturbance?)

What does science say? The studies are divided.

Some studies don't find evidence for PFS. Other smaller studies show biochemical abnormalities in PFS patients.

Disappear The Side Effects of Finasteride?

For the large majority: Yes.

Your hormone balance usually recovers inside:

  • 2 weeks (mild symptoms)
  • 1-3 months (most cases)
  • 6-12 months (hard cases)

Science says, reversible. Some patients say: Not to me.

Who's right?

Both, probably.

Your body is not a machine with predictable outcomes.

What can you do if side effects persist?

Step 1: Stop finasteride (consultation with your doctor) Step 2: Have hormone levels tested (testosterone, estradiol, DHT, SHBG) Step 3: Consider temporary hormone therapy if necessary Step 4: Give your body time to recover

Prof. Bakker, endocrinologist: "The hormone system is complex and adaptive. Usually the body itself finds a new balance after stopping finasteride. But this could take time."

Finasteride Permanent Infertility

Let's tackle this right now:

There's no evidence of permanent infertility.

But finasteride affects your sperm temporarily:

  • Less volume
  • Slightly lower concentration
  • Changed Morphology

Are you planning a baby? Stop 3-6 months in advance.

That's not guesswork. That's medical advice.

The dates in detail:

A systematic review from 2021 investigated 21 studies on finasteride and fertility:

  • Mean decrease in ejaculation volume: 22-28%
  • Deduction in sperm concentration: 21% (temporary)
  • Time to normalize: 3-6 months after discontinuation

A French investigation followed 35 men after stopping finasteride:

  • 31 males (89%) showed fully normal sperm values at 6 months
  • 34 males (97%) normal at 12 months
  • 35 males (100%) normal at 18 months

All participants who wanted a child succeeded within 2 years.

Think of finasteride and fertility as a temporary pause button. Not as permanent damage.

Finasteride And Minoxidil Permanent

The dynamic couples of hair retention. 💪

Finasteride: stops the DHT Minoxidil: stimulates blood supply

Together? More powerful than alone.

A study with 100+ men showed:

  • Finasteride alone: 68% effective
  • Minoxidil alone: 59% effective
  • Together: 84% effective

That's significant.

But permanently? Only as long as you use them.

I saw it on myself. The combination gave me visible thicker hair after 4 months.

The science behind the synergy:

Finasteride and minoxidil work in completely different ways:

  1. Finasteride: Hormonal Route
    • Blocks 5-alpha reductase
    • Reduces DHT production
    • Discontinues progressive follicular damage
  2. Minoxidil: Vascular route
    • Opens potassium channels in blood vessels
    • Increases blood flow to follicles
    • Extends the growth phase of the hair cycle

Together they form a one-two punch:

  • Finasteride stops what damages her
  • Minoxidil gives extra nutrition and boost

Dr. Janssen, hair researcher: "It is like stopping smoking (finasteride) and starting running (minoxidil). Both help your health, but in very different ways."

Finasteride Permanent Hair Growth

"Permanent hair growth" is marketing language.

Reality is more nuanced.

With continuous use you get:

  • Delayed hair loss (almost everyone)
  • Regrowth (approximately 66%)
  • Thicker hair (with responders)

But finasteride is like brushing teeth.

You quit? The effect disappears.

You're starting over? It'll take months to get you back to level.

What determines if YOU get good results?

Key factors:

  1. Start-up age
    • Younger = better (under 35: best results)
    • Above 50: decreased response
  2. Hair loss stage
    • Norwood 2-4: excellent opportunities
    • Norwood 5-7: limited regrowth
  3. Type of hair loss
    • Crown: best results
    • Hairline: moderate results
    • Diffius: mixed results
  4. Consistency
    • Daily use is crucial
    • You can throw back even a week.
  5. Genetics
    • AR receptor sensitivity
    • 5AR enzyme activity
    • Collagen production

The longest study (10 years) shows that those who have positive results after 1 year will continue to benefit up to 10+ years.

Finasteride Permanent Sexual Adverse Reactions

The elephant in the room. 🐘

Let's be honest. Some men report permanent problems:

  • Libido decreased
  • Erectile dysfunction
  • Sensitivity

Where did this come from?

Possible explanations:

  • Continuous hormonal changes
  • Neurological impact
  • Psychological factors

The medical world is struggling with this.

Post-Finasteride Syndrome (PFS) is controversial.

But the stories are there. And they're real.

My advice? Know the signals:

  • Sudden decrease in sexual interest
  • Sustained erection problems
  • Emotional changes

On these signs: stop and consult your doctor. Right away.

An honest look at PFS:

A systematic review from 2023 analysed 29 studies on PFS:

  • Cases reported since 2011
  • Symptoms persistently after discontinuation
  • The most common complaints:
    • Decreased libido (94%)
    • Erectile dysfunction (92%)
    • Decreased enjoyment (89%)
    • Depressive symptoms (75%)
    • Cognitive problems (72%)

What we don't know:

  • Exact incidence (estimates range from 0.1% to 3%)
  • Predictive factors for those at risk
  • Active treatment protocol

What we know:

  • It exists, even if it is rare
  • It can seriously affect lives
  • Prevention is better than cure

René, 35: "I have been using finasteride for 5 years without problems. But if I see anything, I'll stop immediately. My hair is important, but not so important."

Finasteride Permanent Loss of Libido

Can it? Is it likely? No.

The statistics are on your side.

But don't tell those who suffer from it.

For them, it's 100% reality.

What makes some vulnerable? We don't know yet.

Maybe:

  • Genetic factors
  • Pre-existing hormonal imbalance
  • Age in use

You can't predict whether you belong to it.

That makes the choice difficult.

Deeper diving into the mechanism:

DHT is not only important to your hair, but plays a crucial role in your sexual health:

  • Neurosteroid function in the brain
  • Libido regulation
  • Erectile function

In most men: when finasteride stops, DHT returns and normalizes the function.

At a small percentage: permanent changes in:

  • Neurosteroid metabolism
  • Androgenic receptor sensitivity
  • Estrogen: Testosterone balance

Prof. Van Houten, neuropharmacology: "We are just beginning to understand how complex the interactions between hormones and brain chemistry are. Finasteride affects more than just hair growth."

What can help if this bothers you?

  • Testosterone supplementation (in some cases)
  • Adaptogenic herbs (ashwagandha, maca)
  • Regular exercise
  • Stress reduction

The painful truth: science does not yet have a definitive solution.

Long-term debt securities from Finasteride

Let's also discuss other long-term effects you need to know:

metabolic effects

  • Small risk of insulin resistance
  • Possible increased risk of type 2 diabetes
  • Subtle changes in body fat percentage

Bone health

  • DHT plays a role in bone maintenance
  • Long term use: small decrease in bone density possible
  • Increased risk of fractures? Research is ambiguous

Psychological Impact

  • Rare cases of depression
  • Anxiety (especially in pre-existing conditions)

Cardiovascular

  • No proven link to heart problems
  • May affect blood pressure slightly in some patients

Prostate Health

  • Lowers PSA values (important for screening)
  • Decreases prostate
  • Reduces risk of prostate complaints

The 10-year safety study shows: in most men (96%) no significant long-term problems outside occasional sexual adverse events.

The Fair Weighting

Let's put everything on the table:

Yes column:

  • It really works (for 80-90%).
  • One pill a day
  • Relatively cheap
  • Scientifically supported
  • 10+ year safety dates
  • Improves self-confidence

No column:

  • Lifelong commitment
  • Possible sexual adverse reactions
  • Low risk of long-term problems
  • Works only for certain types of hair loss
  • Must be taken continuously
  • Results disappear on discontinuation

I chose finasteride. After 3 years, no regrets.

But I know others who chose differently.

And that's fine.

Best candidates for finasteride:

  • Men under 40 with recent hair loss
  • People with hair loss on the crown
  • No history of sexual dysfunction
  • Geen kinderwens op korte termijn
  • Stabiele geestelijke gezondheid

Minder geschikte kandidaten:

  • Ver gevorderde kaalheid (Norwood 6-7)
  • Geschiedenis van depressie/angst
  • Bestaande seksuele disfunctie
  • Actuele kinderwens
  • Verhoogd risico op prostaatkanker

Tips Als Je De Sprong Waagt

Besluit je te starten?

  7. Be patient.

Alternatieven Voor De Twijfelaars

Niet overtuigd van finasteride? Er zijn andere opties:

Bewezen alternatieven:

  • Minoxidil (5%): 60-65% effectiviteit, werkt topisch
  • Microneedling: Verhoogt absorptie van andere behandelingen
  • Ketoconazole shampoo: Milde anti-androgene eigenschappen
  • Low-level laser therapy: Stimuleert follikels, matige resultaten
  • Hair transplantation: Permanent maar duur

Experimentele opties:

  • RU58841: Off-label anti-androgeen, weinig veiligheidsdata
  • Stemceltherapie: Veelbelovend maar vroeg stadium
  • Exosomen: Opkomende behandeling, beperkt onderzoek
  • Dutasteride: Krachtiger dan finasteride, meer bijwerkingen

Natuurlijke benaderingen:

  • Saw palmetto: Zwak 5AR-remmer, beperkt bewijs
  • Pumpkin seed oil: Milde anti-androgene eigenschappen
  • Rosemary oil: Vergelijkbaar met 2% minoxidil in één studie
  • Natural DHT Blockers: Pygeum, groene thee, etc. (beperkte werkzaamheid)

The Bottom Line

Welke groep jij bent? Onmogelijk te voorspellen.

Dat maakt dit geen medische beslissing.

Het is een persoonlijke gok.

Met goede kansen, maar echte inzet.

Wat kies jij?

Don't forget:

  1. Raadpleeg altijd een arts voor je start
  2. Monitor je lichaam zorgvuldig
  3. Stop bij waarschuwingssignalen
  4. Er is geen perfecte oplossing

The choice is yours.
